Facility-Sponsored Participation Model

The facility elects to participate in the Facility-Sponsored Shared Support Model, under which:

  • A My Homecare & Services caregiver will be present on-site during designated hours

  • Support availability is shared among participating residents

  • Services are brief, task-based, and non-medical

  • Participation does not guarantee service delivery to any individual resident

Participation Fee and Enrollment Requirements

  • Daily Fee: $25 per participating resident.

  • Minimum Enrollment to Initiate Services: 12 residents. Services will only begin once this threshold is met.

  • Expanded Staffing Threshold: 24 residents. Services with two (2) caregivers will only begin once this threshold is met.

  • Participation fees and enrollment thresholds are reviewed periodically and may be adjusted based on service utilization, staffing requirements, and operational feasibility.

  • This is a shared-access service; participation does not guarantee individual service delivery.

  • Fees apply regardless of daily usage.

  • Final participation counts will be confirmed prior to service launch.

  • The facility agrees to meet or exceed the minimum participation requirement.

Service Availability & Expectations

  • This service provides shared access, not individualized or scheduled care.

  • Residents may not receive assistance on a given day due to availability, urgency, or demand.

  • My Homecare & Services does not guarantee that any participating resident will receive service.

Operational Boundaries

The facility acknowledges that this service:

  • Is non-medical

  • Does not include nursing or medication administration

  • Does not provide continuous supervision

  • Does not replace facility emergency response protocols

In the event of an emergency, caregivers will follow facility procedures and notify staff immediately.

*Once the program begins, a caregiver will make reasonable efforts to check in with each participant to confirm participation and needs each day.

Services will only begin once the minimum enrollment is met
