Service Overview Acknowledgment

My Homecare & Services provides a Shared Support Service in assisted living communities. This service offers shared access to an on-site caregiver during designated hours to assist enrolled residents with brief, non-medical needs as they arise.

By enrolling, I acknowledge and understand that:

  • Services are shared among enrolled residents

  • Assistance is task-based and brief in duration

  • Individual service time is not guaranteed

  • This service does not replace personal homecare services

  • This is not an emergency response service

Service Hours

I acknowledge that service hours are currently:
7:00 a.m. – 3:00 p.m.

Daily Participation Fee & Enrollment Model

I understand that this is a shared-access service, not hourly care.

  • Enrollment begins at the $50 per day participation rate

  • Fees are based on total resident enrollment

  • As additional residents enroll, the daily fee per resident will decrease

  • Fees apply regardless of daily usage

  • Individual caregiver availability per resident may decrease as enrollment increases

Fees & Discontinuation

  • A $50 non-refundable registration fee is required and will be applied to service charges

  • Two (2) weeks’ written notice is required to discontinue participation

Important Limitations Acknowledgment

I understand that this service:

  • Is non-medical

  • Does not include nursing or medication administration

  • Does not provide continuous supervision

  • Is not an emergency response service

For emergencies, facility staff and emergency services will be contacted.
